At CAN-AM, protecting your health information is of utmost important to us. Click below to view our HIPAA Patient Privacy Statement. If you need us to share your records, please fill out the PHI release below and bring it with you to your next visit.

Insurance & Billing

IMPORTANT UPDATE: 

We currently enrolled and accepting Anthem BCBS, Aetna, CareFirst and Medicare insurance plans! Please submit your insurance card with your initial consult intake paperwork or bring in your insurance card for acceptance verification.

If you are not covered by an insurance plan we are enrolled with but would still like to seek care with us, we are happy to assist patients submit claims to their insurance for reimbursement by providing super-bills for provided services so they can still take advantage of their chiropractic benefits. 

We also accept credit or debit cards, cash or check as acceptable forms of payment! 

Patient Paperwork

Upon scheduling, patients will be prompted to fill out required paperwork through their patient account with Jane app.
